About Foodie Talent Agency

I understand by filling out this form and clicking the send button that I am agreeing to be represented by Foodie Talent Agency on a non-exclusive basis. I am giving the agency permission to use my resume, bio, information received during interview, photo and or likeness. This information will be used by the agency to secure me a job. I understand that the agency does not guarantee me work. In the event i am contacted by prospective employers i will make myself available by phone and or email to pursue interviews resulting in my being hired. I agree that if i am hired that i will contact Foodie Talent Agency within 5 days and inform them of my status. From time to time i will also update them if i am hired by another outside source or our employers. I understand that I am not allowed to disclose any confidential information provided to me by the agency and may terminate this agreement by email with 5 days notice.
